I read it that they fitted a Di2 Derailleur, so selected the Electronic Derailleur in Emax, giving them Autoshift, as you cannot do this with customer copies of Shimano ETube.
I took a closer look. It was just a twisted derailleur and bent hangar. Must have hit something on the trails.
I have a hangar alignment tool, so I fixed that. Then just straightened the twisted derailleur cage. Re-setup the derailleur alignment. And all is silent again.

Yes it's intermittent. It only creaks under high load pedal forward. Shimano Mid drive motors don't spin the chainring when you pedal backwards. So nothing happens if you pedal backwards.
If the roll the bike backwards. You could get some noise. But that is more to do with your chainline...
